Fostering Hope Initiative

Catholic Community Services’ Fostering Hope Initiative provides resource navigation and support with the following:

  • Self Sufferance support
  • Case Management 
  • Oregon Health Plan /Marketplace Assisters
  • Parent Education
  • Birth Doula/ Pregnancy Services 

Fostering Hope Initiative partners with the Oregon Department of Human Services to provide Family Support and Connections (FS&C) services in Marion, Polk and Yamhill Counties. OHSU School of Nursing

The Interprofessional Care Access Network, or I-CAN, is a program within the OHSU Interprofessional Initiative and housed at the School of Nursing. By participating in this program, students gain a deeper understanding of the social context of patients with complex health needs. I-CAN’s primary purpose is to provide a model in which health professional students are in the community to address the complex health needs of vulnerable and disadvantaged individuals, families, and populations.

Oregon Department of Human Services

Oregon’s Self-Sufficiency Programs (SSP) provide resources, like cash and food assistance, as well as targeted services to help people move out of poverty and​ toward whole well-being. ​Programs include:

  • Food benefits (SNAP)
  • Cash Assistance (TANF)
  • Child Care Assistance (ERDC)
  • Services for domestic violence survivors
  • Resources for refugees
  • Support for youth experiencing homelessness.
Polk County Family & Community Outreach Department

The Polk County Family & Community Outreach Department hosts a wide variety of programs to support the community. These programs include:

The Resource Center assists families and individuals, helping to meet their basic needs and supporting them in achieving self-sufficiency. Services include:

  • Emergency Food Boxes
  • Hygiene Supplies
  • Community Mailbox
  • Diapers and Formula
  • Emergency Financial Assistance

Veteran Services Office works to ensure that Polk County veterans and their families receive all benefits provided by federal and state law. A veteran service officer is the advocate for veterans and their families. Services provided by appointment only include:

  • VA Claims/compensation
  • Surviving Spouse Benefits
  • Burial, Marker, & Flag Applications
  • Dependent Child Benefits
  • Voc. rehab Applications
  • VA Medical Enrollment
  • Locating of Records

School-Based Mental Health (SBMH) works in partnership with local school districts. SBMH offers a wide range of services, including:

  • Classroom presentations on character education, anti-bullying, problem solving, suicide prevention, and social skill development
  • Individual emotional support
  • Group and individual skill building
  • Crisis support
  • Parent conflict resolution
  • Youth and parent support groups

Resource Navigators provide ongoing case management support to families referred by the Oregon Department of Human Services.

Homelessness Prevention and Outreach works to prevent and address homelessness for clients in Polk County. Services include:

  • Street Outreach
  • Connections to shelters and other resources
  • Case Conferencing
  • Partnership with the ROCC, DHS, Behavioral Health, Parole and Probation, Police, and more
  • Connection to MWVCAA ARCHES

Early Learning and Family Engagement program supports families in connecting with resources, preparing children for kindergarten and providing evidence-based parenting education. Activities include:

  • Family Fun Events
  • Parent Education Workshops
  • Professional Development for Local Professionals

Drug, Alcohol, Problem Gambling, and Suicide Prevention work to reduce and prevent the negative effects of drug, alcohol and gambling addiction through a variety of strategies including:

  • Positive youth development
  • Evidence based parenting classes
  • Community assessments and surveys
  • Media campaigns
  • Supporting policy development
  • Education and training
Polk County Public Health- WIC Program

The WIC program serves more than half of all infants born in the U.S. Whether we’re educating families on nutrition and breastfeeding, providing them with free and healthy food, or referring them to outside care and social services, we hope to give them the resources, knowledge, and tools they need to be the moms they want to be. We are not only here for women. We are here for all parents, caregivers, and moms-to-be who meet certain income or nutrition-risk requirements. When you refer someone to WIC, you ensure their access to healthy options and communal support.

The Polk County Public Health-WIC Program provides services including:

  • Nutrition Education
  • Breastfeeding Support
  • Healthcare Referrals
  • More

Families must be income-eligible women who are pregnant or postpartum, infants, and children up to 5 years old

Polk Community Free Clinic

Polk Community Free Clinic provides free quality compassionate health care to the uninsured and the underinsured people in our community.

Services that the Free Clinic CAN help with:

  • Acute Problems
  • Chronic Diseases
  • Medications
  • Mental Health
  • Women’s Health

Services that the Free Clinic CANNOT help with:

  • Emergencies (call 911)
  • Narcotics or other Controlled Substances
  • Work or Accident Injury Claims
  • STI Testing

Polk Community Free Clinic is hosted at the Resource Center on 2nd and 4th Saturday each month. To make an appointment, call 503-990-8772. Walk-ins are limited to provider availability.

Small Town Pediatrics

Dedicated pediatric care including well child checks, chronic care management, ill/acute care. 

Small Town Pediatrics is proud and excited to finally be able to serve the families of Polk County. Not only will these families no longer have to drive to another town for care, they will be part of the Small Town Pediatrics community. This means never worrying about your child alone and having direct access to the clinicians who care for your children.

West Valley Housing Authority

West Valley Housing Authority provides safe, decent, and affordable housing for those in need; while encouraging their progress toward self-sufficiency.

The West Valley Housing Authority provides assistance to nearly 1,100 households through our administration of:

  • The Housing Choice Voucher (HCV) Program (also known as Section 8 program) which allows us to help up to 703 households by providing housing assistance payments, to private landlords, on behalf of the household in order to make the rent more affordable. 
  • The Public Housing Program which allows us to offer our 373 rental units to low income families and / or individuals who may be elderly or disabled at rents that are affordable.
  • Our Affordable Housing Program consists of 8 rental units, we own and operate, that we provide as affordable housing for households here in Polk County.
WorkSource Oregon

WorkSource Oregon is a statewide partnership with the Oregon Employment Department and state, local, and nonprofit agencies. We provide a variety of employment and training services to job seekers and employers in Oregon including:

  • Job skills workshops
  • Job search assistance
  • Resume & Interview Support

WorkSource Oregon helps people find jobs and helps businesses find talent.
